Telemedicine consultation Registration and price list of telemedicine

To book a telemedicine consultation:

1. When registering by phone or e-mail, specify that the service will be provided in the form of telemedicine consultation (telephone conversation) and confirm your telephone number.
2. After registering for the indicated teleconsultation date, pay the fee according to the price list. Go to the price list
3. Bank transfer details:

   Transfer title: “Full patient’s name, teleconsultation on …….”
4. Send the confirmation of the transfer to the address: teleporada@gizinscy.pl at the latest one day before the scheduled teleconsultation.
5. On the day of the scheduled teleconsultation, the doctor will contact the patient at the telephone number previously indicated during registration.
